Our providers may participate individually in various other health insurance plans. Please contact your insurance company to determine if a particular clinician is contracted with your plan.
In order for us to determine benefits, please have your insurance card available when calling to schedule your appointment. We will need information found on the back of your card to verify your benefits and obtain authorizations.
In addition, it is necessary for you to bring your insurance card and driver’s license or other government issued identification card to your first appointment. These documents are needed to verify your identity and protect you from insurance and identity fraud.
Payment is required at the time that services are provided. We accept cash, checks, and all major credit cards. A security deposit is generally required for all new patients in case of a ‘no show’ or late cancellation. If we are contracted providers with your insurance company and have confirmed your coverage, you will only be responsible for your co-payment, co-insurance, and/or deductible at the time of your visit.”
Please be aware that verification of coverage by the insurance company is not a guarantee of coverage or payment for services. You will be personally responsible for all non-covered services. It is up to each patient to know the benefits and coverage limits of their insurance policy.
